How to Apply for the Medicaid Waiver in Indiana

Step 2: Complete and Submit the Application

  • Submit the application to BDDS either online, in person, or via mail.

Step 3: Schedule an Assessment

  • Once your application is received, BDDS will schedule an assessment to determine your level of need.

  • You or your caregiver will be interviewed about your daily needs and required supports.

Step 4: Wait for Eligibility Determination

  • If approved, you will be placed on a waitlist for waiver services unless deemed eligible for immediate assistance under the CIH waiver.

  • You will receive a notification about your eligibility status and next steps.

Step 5: Develop an Individualized Support Plan (ISP) now called Person-Centered Individualized Support Plans (PCISP)

  • Once funding becomes available, you will work with a case manager to create an Individualized Support Plan (ISP).

  • The ISP outlines the services you will receive and the providers who will deliver them.

Step 6: Begin Services
